Go to setup in version 5.9 and click on sheets at the bottom. Highlight Rx. Click on copy and it will show in the box below. Double click on it so that you can edit it. When done, save it. Now it will be substituted for the old Rx form. There is info in help section on Sheets.

I don't know of any service that just does what you are wanting. Insurance answers plus includes as many fee schedules in their program as they can get, and will attach it to the employer plan.
