Open Dental Forum

To date I've been using paper forms (new patient info, med hx, etc) and scanning them into images. I'd like to eliminate that paper, scanning, and time waste. Are the webforms pre-made by Open Dental for our use? Can we, the users, tweak and alter the forms to our liking? Or do I have to make my own...
