I don't participate with a certain Insurance companies lower fees plan, but do par with others of the same insurance co. For the one plan I don't par with, the patient pays in full and assignment of benefits go to the patient, what fee schedule would be best for this arrangement?
I give a prepayment discount for payment in full---is there anyway to include this on the eclaim form e.g. 5% prepayment discount

You don't participate, so it's a category percentage plan. Fee schedule for the plan would be your standard office fee schedule.

I do that because legally I have to report what I charged the patient, otherwise it could be construed as fraud. The insurance company expects you to give them the same discount, otherwise you are misrepresenting your fees to them.
So is there a remarks database for the eclaims where I can enter such phrases as X% discount given or subgingival decay necessitates buildup etc....
