I need a query, please

I need a query that will tell me the number of claims that have been paid versus denied for a certain procedure code (or multiple procedure codes that I can type in) by a particular carrier for a particular provider in a specific date range.

A certain company is questioning my diagnoses and denying claims. I need to know how many were paid and how many were denied. It has to be provider specific.

I've checked the query page, and can't find what I'm looking for.
