We are having trouble with our patient registration web form. The patient portion works fine. But the subscriber information for patients with insurance is not properly imported into OD.
They only problem we can see is that there isn't a field for subscriber last name only patient last name. There is a field for subscriber first name. Sometimes the last names are different.
Are we missing something here? How do others get this form to work properly?
Thanks

Re: Subscriber last name on web forms
If you look at the screenshot of the import window, the fourth screenshot at
http://www.opendental.com/manual/patientform.html
you will see that it is not possible to import insurance information. That window is enormously complex as it is, and trying to automate the insurance was just becoming unmanageable. That's why all the insurance fields are in red, to remind the staff of what they have to type in. We will improve it over time. For example, in the subscriber row, we will probably add a [double click to pick], just like we have with the referredFrom field. And then we will add way to pick an insurance plan based on what the user typed in.
