You can create a new FORM, ALL FORMS ,a treatment plan consent , ETC... (using SHEETS)
and email them to your patient from inside OD, and all is FREE , INTEGRATE WITH OD
YOU NEED TO CREATE A TREATMENT PLAN CONSENT FORM IN SHEETS FIRST.
When you click on "Consent" tab in the treatment plan, many did not know that they have to create a treatment plan consent form in OD in adavance, FOR THE PROCEDURES HIGHLIGHTED, to be available in a " treatment consent form., so the patient can sign it.
There is no treatment plan consent form in OD Templates.
Create that form in advance to be able to pick it from the drop down menu of the consent tab.
When you design the treatment plan consent form in SHEETS, it must include the treatmentplanProcs or treatmentplanProcsPriority
Static text field.
When a treatment is planned, highlight the procedures, click on consent and email it to the patient or send it to the kiosk.

OD comes with many forms, webforms, already installed and ready to be used immediately and you can put them on your website immediately for your patients to use

CUSTOMIZING SHEETS
It's a lot like making a fillable PDF. I think they modeled sheets editor off of Adobe.
To customize the form, we created the form in a word document and converted the word document into an image to use it as a background.
We then imported the image of the form into open dental. Now, with the sheets option, we use the image as a background and you can add Name box, addresse box, check boxes, signature, etc... to each sheet. It’s definitely time consuming but worth it.

FREE GOOGLE FORMS
In addition to these consent forms many people use the FREE Google forms to make their own Medical History Form, Registration form, and the screening form.
Google forms work on Tablets, Cellphones, ipad, Desktop....etc.....anything that has internet

Eclipboard feature is different than the kiosk feature.
1- For the kiosk option you can use only a tablet that has windows installed on it, and you need to install OD on it. You dont need to sign up for any of OD services. FREE . Surface or Surface Pro tablets are recommended, and a large screen.
2- For webforms, they are included in your monthly Support fees. No additional charge
3-For the eclipboard option you can use almost anything that has internet on it, and you need to download the eclipboard app. You need to be on support and sign up for OD E-services
It may be time consuming to create these forms, but it pays off at the end. Once you are familiar with it, it will be much easier to work with
