adding patient info to website

I would like to add portions of the family/account pages to my website which is restricted by password to referring docs. Ideally, I would lile to copy the entry that is emailed to me directly into OD for New Patients. This would also include a jpeg image of a pano. It would be a form kinda like ordering something online and if something is not filled in the email will not send from my website. So often different staff are sending the information and forget the phone number or DOB etc. Is this something that could be easily done? Sure would make my job easier not having to type all that info in again from outlook. I can not write code but I am sure a couple of my friends are familiar with C#

You could put a pdf on your website for them to fill in.
