We had patients to fill out registration, medical and other forms through kiosk manager. I thought the filled out info would automatically be entered in the pt's chart, but it didn't.
I entered the pt's info manually, but can't figure out where to enter medical history info. I did found where to place the info about drugs pt takes. However, can't locate where I can enter the rest of the info from the medical history form (Name of medical Dr, Emergency contact, Tobacco use, Reason for today's visit, Information about past Xrays). Or is it something the Doc enters manually in the notes?

To import patient information (from the Registration Form):
click on Forms | Select Registration Form | Import
Medical History does not get imported. You can either copy relevant medical history over to the Chart Module, or click on Forms | Medical History every time patient comes in

Progress Notes is one of the grids within the Chart module. Filled-out forms such as medical histories will also show in Progress Notes, so that would be another place to find them in addition to the Patient Forms window. We did just add medical history import for version 12.3. But many of the fields you listed are not formalized fields in Open Dental, so there's no place to "import" them to. You can either leave them in the form, or you can transfer them to a more visible note. I would suggest the large medical history box inside the medical window. For fields that you want to formalize, I would suggest custom patient fields:
