Hi Dr. Sparks,

I noticed 2006 claim form was introduced starting from 4.9.x. However, if you were to go into Open Dental and edit the claim form, the background image is not showing up. It's referencing a file named "***2006.gif" but I cannot find this anywhere in the installer or source code.

It's not in the source code. It's copyrighted, so we can't include it. The only way to get it is to use 5.0 and to be on a support contract with us.
