Bug: Insplan Filing Code Subtype

When adding a new insurance plan for a patient using "Pick from list", the Filing Code Subtype is not being populated. Therefore when the new plan is saved for the patient it is clearing the subtype for all identical plans, where there used to be one. Using version 7.4.16, but I think this has been happening for a long time.

This should be a non-issue in 7.5 because picking from the list actually picks an existing plan rather than just making another copy of it.
