I would like to customize the look layout of the Statement (Under Accounts->Statement). I am upgrading a practice and integrating various systems. I need the Statement that gets printed to the patients to be consistent.

• Is there a way I can create my own page design?
  Can I control how the data is displayed / where is appears on the page.
  What part of the code/Open Dental do I go to change the settings for this?

I am trying to mimick the design of statements like those you see from your cell phone/telephone company, or even credit card statements for Open Dental.

That's feature request #5.

It was brought to my attention by a patient that the bottom portion of the statement doesn't have the practice name, address or phone #. That information is only on the top portion, which they send back with their payment. Until the feature request for customizing the statement is complete, could that information be added to the bottom portion?

That's feature request #1133, and it's much easier for us to add.
