4- Google forms, Free, can be completed on any device that has internet on it, our patients complete them on their cell phones.

7- To customize the sheets/forms in Open Dental, and to match your office designs, you can do it by using "Sheets", or there are many users available on Open Dental Facebook group that charge affordable prices to customize these sheets/forms for you.
It takes time to learn how to create SHEETS in OD, but once these forms are created, your staff will love you.

What kind of Windows device do you use in your office for patients to put info in? A Surface tablet?

For the Kiosk option, you can use Surface Pro

For Open Dental e-clipboard, Google Forms, other vendors plugins, you can use any device that has internet on it, tablet, iPad, cellphone, etc...

In my office we use GOOGLE forms.
Our patients complete them on their cellphone. We text them the link.

Did you ever get full sync into OD with the Google Forms?

There is no free plugin to sync Google forms. We manually enter it.
This normally work well if you have a small office. For large offices, Open Dental e-clipboard option will work well

For small or startup offices, follow the link below, here you can find some free or low cost plugins for your office. You will need someone with some computer knowledge to do the installations.
