Query Help

I've looked around a bit (maybe 5 mins) but I can't seem to find a query for what I need. Can someone help?
Collection (or net production) by appointment over a given date range with following categories
- Hygiene codes vs treatment (would be nice to separate out by provider as well)
- Would be nice to have total appointments, but then have an average collection for each appointment by provider
Number of days worked by provider in a given date range. Basically could be set up to count a day if a procedure was completed under the doctors name. I see one for total days worked by a practice but it isn't split by doctor.
