One of the Queries at the bottom of the User Query Favorites window is "Table - Insurance Template" .
When I run it I get "Invalid query: Table 'opendental.instemplate' doesn't exist."
What is that table supposed to return?

Query favorites are just SQL queries that have been saved for later.

The InsTemplate table does not exist in the Open Dental schema, so the most likely source of that User Query Favorite is a 3rd party.
