Will the document upload via SFTP be scheduled to be uploaded after API call, as in, will the API call return response right away or wait till the doc is uploaded?

I am searching for patients by last name. When I search for "Smith" (this is in my local demo DB), the API returns empty set. When I search "Test", I receive a list of about 30 patients, who seem to be someone else's test patients. Seems to me API doesn't "see" my local OD instance? 342 John Test 00...
