Interesting circumstance today. We are working through 4 quads sc/rp on a patient. By mistake, the first appt. was entered as a 4341(sc/rp) instead of 4355 (debridement).
I changed the procedure code, but not the note and no entry was shown in the audit log (chart). It does however show in the audit trail. My (smart) front desk noticed that the fee ended up changed and this threw off the day sheet for (I believe 1123/09).
So what would be the best practice for something like this? Should it show up in the chart audit log?

The audit in the Chart is mostly geared towards preserving the notes. It shows all history of all notes and it also shows deleted procedures because they might have attached notes. It did show up in the audit trail. We are always thinking about ways to make the audit trail more powerful, and this nuance will be taken into account.
