Medical Claim Form HCFA 1500

I tried to print a medical claim form for our patient today. HCFA 1500

1-The subscriber Id number was not printed on the claim form, even I did enter it in the insurance section.
2- and the year in the date of service on the claim form was " 1 "
3- And if I try to "edit the procedure" to enter the ICD 9 code, The area to ad the ICD 9 is not being displayed as it shows on your website.
I could not enter the ICD9 code

I use OD version 7.1.27.0

1. I'm not sure. Maybe check the setup. See #2 below.
2. I think it's just getting cut off. Make it wider in the claimform setup (the interface is not exactly easy to use).

I got to SETUP/ Claim Forms/ HCFA 1500/ and I added the subscriber ID to the top of the claim. it prints now.
2- and the year in the date of service on the claim form was " 1 "
For the date of service I changed the width from 80 to 84. it prints better.
3- And if I try to "edit the procedure" to enter the ICD 9 code, The area to ad the ICD 9 is not being displayed as it shows on your website.
From SETUP, Show Features, I checked the Medical Insurance , and the ICD9 option is being displayed,

BUT
it does not print the ICD 9 Code that I enter.
It always print "1" and it does not also print the MODs

It shouldn't be this much work. Maybe you have an old version of the form? Did you import the form, or was it already present? We might have left an old version of the form in. Maybe try to import the one that's posted online? If that fails, maybe call us and we can get it working for you. I don't think it will ever print MOD because that's not medically relevant.
