After conversion, our office ended up with lots of duplicate insurance plans, lots of outdated plans, carriers, etc.
My questions: When is it safe to combine? When should they be dropped? When should they be hidden?
What if there are no current subscribers? What should be done then?
And what advice should I follow with Insurance Carriers vs. Insurance Plans? IOW, do the same guidelines apply?

I would say combine liberally. I would start with carriers, because there are fewer fields, and combining carriers will automatically combine a number of plans. But if the phone numbers are different, I think they should be left separate. Dropping is always safe, but that's not any sort of bulk cleanup; just one account at a time. You can always "undrop" them. Hiding should be for ones that you don't want to let users pick from when assigning to new plans. I can't think of any clear guidelines for using that option, but it does come up.
