I have several patient charts that had progress notes with a status of complete that are now missing. The only change that I am aware of is that the appt. proc quick add were modified. Some were hidden and some were re-written. Would that have anything to do with it?

No, that would not "delete" any progress notes. Please understand that it is impossible for a user to delete any progress notes. All changes are recorded and preserved. In the Chart module, go to the Show tab and check Audit mode. Then look for your missing notes.

I did what you said, and it does show the auto note attached for the procedure, however, the additions from the assistant are still not there. I also found out that the appointments with the missing notes were set complete the day of service, but when they go back a few days later, it is no longer set complete.
I am sure we are looking at a setting or a process issue, however, because we are so new to Open Dental we need help figuring this out!

Is your network setup standard or are you using something complex like daisy chain replication? In other words, is the database only being used for your location, or is it being shared by multiple offices?
