? – what is the best OPTION for having MULTIPLE offices

I am a network engineer working with a practice that is using OD. They are expanding to have 3 remote offices in addition to the main office. I am looking for the best option in order to deploy OD in the additional offices.
1. My first thought is to use a Citrix Server in the main office and have the remote users (from the remote practices) connect via a VPN (either dedicated data circuits i.e. T1 or high speed Internet connections). What are the advantages and disadvantages of using Citrix? As a simple test, I used remote desktop over a test VPN I created and it seemed to work “OK”.
2. Should I deploy a different OD SQL database at each location. How do i "sync" databases, are there any built in tools for "updating/sync database" or is there a way to run a central database in one location and run the application on a server at each remote location?

I would appreciate any advice on the options available. Below are the requirements.

Share and update a central database / or share the database(s)
Main Office – 7 simultaneous local users
Remote Office 1 – 5 remote users
Remote Office 2 – 5 remote users
Remote Office 3 – 2 remote users
