PPO Fee bug

2 bugs...one duplicated and 1 not sure of:
1)
I think I have isolated a bug in the PPO Fee calculations. When you change an code (like surfaces on a filling) the PPO fee is inserted rather than the normal fee.

2)
It seems that sometimes it puts in the PPO fee instead of the standard fee on some new tx as well. I try it again and it works fine so I have not been able to isolate the cases. PPO Percentage is selected in the ins plan & the proper PPO fee sched selected on the plan.

I found one case where I didn't have PPO Percentage checked in the ins and that was the problem, but can't seem to duplicate this problem above where it was selected and should have worked but didn't.
