Change Prov Prompt

I found a problem with the new prov change prompt when moving appts. It shouldn't ask if the provider is the same and it is doing that right now, and also it should have a yes/no rather than yes/cancel. I can make these changes if approved.

We use the OK/Cancel very frequently for this type of situation, and I'm OK with that. As for the provider being the same, it's just as easy for me to do it since I have to do it for multiple versions. Thanks though.
