Work or School Excuse

self-explanatory, this is of a "patient form" type and has a signature box and place for the name of the escort as well as the populated pt info.

I am not too familiar with Sheets and Forms so can someone explain why this document is better formatted as a Patient Form and not as a Patient Letter?

you can't use check boxes or input fields on a letter which you can with a pt form. You could still do it as a letter as the entire letter is editable, where as only input fields are editable on a pt form, I do like using checkboxes though!

Is it just me but all I get from any of these attachments this is source code.

Are you importing them under Setup - Sheets? That is how it should be done, but if you already are, maybe it is a version difference?
