New Patient Tracking

Is there a place in Open Dental for me to input how patient found the practice or referred by whom? It would be nice to run a monthly report to track where and how new patients are entering practice. On the flip note, is there a way to track patients leaving the practice? Thank-you.

As for patients leaving the practice, this can be nearly impossible, because they never call you and tell you that they decided not to come back to your office. Instead, they just silently don't reply, and you can't differentiate these from the procrastinators. If another dental office calls you to get records, then you know for sure you lost the patient. In that case, you could enter that dentist as a "referred to" and check the box for Transition of Care. I don't think anyone ever actually does this.
