Smile Reminder Forms

Can upload as PDFs that can be electronically filled out offsite by patient or in office on ipad or the like it seems. Looks to me like it might be easier way to go than the webforms on OD although haven't delved too deeply yet. Access for patients through the SR Portal. Curious if anyone is using this at all?

Wow,
This can also be used for various other purposes also like patient feedback forms etc
Why has this not been thought on or even implemented
For the most part ,this thread has gone totally unnoticed
