Report for insures patient payments

I had this report before but can't seem to put my fingers on it now.
I need a report for 2014 or a time period consisting of what patients with a specific insurance plan actually paid our office by date or summary.

Example:
Patient...........Mr. Thisa Test
Delta insurance..Delta Dental
2014-02-03......$100.00
2014-03-05......$50.00

I know it is out there somewhere but dang if I can find it.
Any help would be greatly appreciated.

As luck would have it I just discovered query example #93 modified slightly was what I wanted.
